Substance Use Evaluations are designed to measure the frequency and severity of substance use. In completing this evaluation, individuals have taken their first step in the process of recovery. When an individual meets with a licensed alcohol and drug counselor, they are thoroughly evaluated on a holistic level to help create an unbiased report of their use. Be Kind offers evaluations in the client’s home, out in the community or at our Be Kind office, depending on the comfort level of the individual. Our flexibility helps to eliminate any barriers with transportation.

Need a Co-Occurring Substance Use Evaluation? We have available providers to complete this portion as well. Please indicate on your referral form that you are needing a Co-Occurring Substance Use Evaluation so we can get your referral started appropriately.

Evaluations are available at convenient times for the client. Weeknights and weekends are available by appointment only. Completed reports are sent within 10 business days of the evaluation. When making a referral please be sure to complete our necessary form below in order to receive a call for scheduling. For these specific evaluations these are your payment options: Self-Pay Rate: $273, Medicaid: Nebraska Total Care, UHC Community Plan, Healthy Blue and Nebraska Probation Vouchers.
