Here's how we work: As soon as we receive this completed inquiry form, we will email you a link from our secure Therapy Portal, which, upon clicking, will prompt you to choose a password and will then take you to where our required client forms must be completed and signed. Please

make sure to check your junk/spam folder, as the email from Therapy Portal sometimes gets lost in these areas. Our scheduling team will contact you as soon as we have your completed and signed BKC Consent Forms, History Form, and Insurance Form. Completing these three

specific forms will quickly lead to our scheduling team contacting you, as these forms initially provide us with your consent as well as necessary information in order to match you with the best-fit therapist at our practice. Please be sure to list your preferred way of how you would like

our scheduling team to contact you, below. Our scheduling team will identify the best-fit therapist for your concerns and will provide you with their soonest available intake appointment times. For our scheduling team to complete the last step in scheduling and provide the client

with confirmation of the appointment, ALL of the requested forms in the Therapy Portal must be completed by this time.

Insurance: We accept all Nebraska Medicaid plans, which include Nebraska Total Care, United Healthcare Community Plan, and Molina. We also accept commercial insurance plans, including Blue Cross Blue Shield, Anthem, Wellmark, Aetna, Imagine 360, Midlands Choice, Auxiant, GEHA, Firefighter's Union, and Redirect Health. If you don't see your insurance here, feel free to email our insurance specialist at admin@bekindomaha.com to see if it's something we can accept.


Please note: We are unable to take on any new clients with commercial UHC, UMR, or Medica.
